Through Service Coordination, we work alongside participants, families, and providers to arrange services, manage care needs, and make sure the right supports are in place.

We serve a diverse community and offer support in multiple languages. If English is not your preferred language, we will do our best to connect you with a Service Coordinator who can communicate with you comfortably and clearly.
